Primary Care - Establish: Normally Completed via Telehealth (45-60 Minutes)
Andrea will go over your intake questionnaires prior to your visit and then meet with you to talk further about your concerns and goals. You may be asked to provide additional information such as previous labs or visit notes from previous appointments to gain a more complete picture of your current state of health.
All new patients to RMHP must complete this initial intake appointment in order to establish care with Andrea.
